If you schedule an appointment with me, I'll ask you to download and complete this form and return it to me. You can scan it and email it back, or send it regular mail if there is sufficient time. Payment: Thank you for making payment in full at time of service. Phone sessions are to be kindly prepaid through this site via credit card or PayPal or by check received by and made out to: Tapping Solutions, LLC, 4742 42nd AVE SW #455, Seattle, WA, 98116.
Insurance: I'm sorry that insurance does not cover the cost of EFT at this time.
Rescheduled and/or canceled sessions: My appointment times are valuable and irreplaceable, therefore please make note of the following:
- If you need to reschedule a session you may do so at no charge as long as you contact me by phone or email a minimum of 48 hours prior to the scheduled session. Example: Your session is scheduled for Wednesday at 1 p.m., PST - so you must contact me by the Monday prior at 1 p.m. PST to reschedule without being charged.
- If you need to reschedule anywhere within the 48 hours prior to our session, you will be charged half of the session fee. Example: Your session is scheduled for Wednesday at 1 p.m. and you contact me AFTER 1 p.m. the Monday prior. Half the session fee is payable. (I reserve the right to make exceptions in the event of actual emergency, on a case-by-case basis.)
- Appointments that are missed without explanation or contact are payable in full.
Refunds: In the event that your experience fits the criteria for a refund of the first session fee as described on the "Fees and Guarantee" page of this site, your money will be refunded. There are no refunds on teleclasses; however, if you are disppointed in any way I request that you tcontact me and let me know why you are dissatisfied. Requests will be considered on a case-by-case basis. Our goal it to provide extraordinary value and to create long-term, satisfied clients!
